Out-of-Town Consultations
At the Tarnow Center For Self-Management, we offer Self-Managementsm treatment packages especially customized for out-of-town patients. The Out-of-Town Evaluation Packages give patients from afar an opportunity to receive the same personalized care we provide our in-town patients. We will administer comprehensive testing and assessments according to the issues at hand. An individualized treatment plan and recommended next steps to follow after returning home will be provided. In addition, Dr. Tarnow and team will consult with your physician to ensure continuity of quality care.

Package One:
May be completed in either 2-3 days dependent upon testing needs, time constraints, client capabilities, and other factors.
Day 1 - Full Day
- Includes initial appointment with clinician (patient and parents)
- 3 – 6 hours of testing
Day 2 - Full Day
- Additional testing hours, if needed
Day 3
- 1 to 1-1/2 hour(s) visit with clinician(s) to review testing results and recommendations
- 1 hour appointment with psychiatrist for treatment plan formation and medication consultation, if indicated
Package Two:
May be completed in 2 days dependent upon testing needs, time constraints, client capabilities, and other factors.
Day 1 - Full Day
- Includes initial appointment with clinician (patient and parents)
- 3 – 6 hours of testing
Day 2
- 1 hour feedback session with clinician to review testing results and recommendations
- I hour appointment with psychiatrist for treatment plan formation and medication consultation, if indicated
